How to make a claim

How To Claim for Outpatient treatments

Your claim form is important as it gives us the information that we need to process your claim .if it is not fully completed we may have to ask for more information

When you visit a doctor, dentist, physician or specialist on an out-patient basis, please settle the bill with them and claim back the eligible expenses from Medmark by provide the following: 

• Claim Form mentioning the member name and membership number. 
• Medical report with the exact signs/symptoms and diagnosis 
• Original stamped breakdown invoice clarifying date and cost of each performed service(s)  with a proof of payment (payment receipt – credit card receipt) 
• H.E the ambassador authorization.
• To  Fill in the payment details section on the claim form shown in the below file. Kindly arrange to send all the completed above documents within six months from the initial treatment date at our office Medmark in Cairo at 44 Abdel Moniem Riad Street-Mohandessin
Pre-authorization

You must contact us whenever possible before inpatient or day case treatment for Pre-authorization .


This mean that we  can confirm to you and to your hospital that your treatment will be covered under your plan.


Pre-authorization puts us directly in touch with your hospital so that we can look after the details while you concentrate on getting well.
