Dear Member, 

You are welcome to submit your claim online by completing the form below. This service is only available to members residing in the countries (cities) mentioned in the form below.

1. Please upload scans of  your completed claim form including the diagnosis.
2. You will need to upload scans of a fully itemized invoice, any prescriptions and any medical reports related to the treatment.
3. Scans of proof of payment (receipts, credit card slips etc.) are required.
4. Kindly make sure that payment details section is completed so that you can be reimbursed without delay.

Once the claim has been submitted you will receive an e mail as a proof of submission

• Scanned documents are only supported in PDF format, with a maximum of 20MB per claim.
  
• Please note that it is mandatory to keep your original bills for 6 months or until the successful the processing of your claim(s) whichever is Later.
